Prior to Visiting Our Office

In your appointment confirmation email you will have received a link to fill out our new patient intake questionnaire. Please fill this out as thoroughly as possible. If you are unable to complete the form online, we will have you fill it out when you arrive at the office. Please arrive 15 minutes prior to your appointment time to fill out the additional paperwork necessary so that we can begin your appointment promptly at your scheduled time.

Please arrive 15 minutes early to your first visit to so our front desk staff can check you in and collect any additional paperwork or information needed. This will ensure that we can start on time so you will have the full amount of time scheduled with the doctor for your appointment. Once the doctor brings you back, we will begin with an initial consultation where we review your health history and get an overview of what brings you to see us.  We will then proceed with an exam that includes orthopedic, neurologic, and functional muscle testing. Following the exam we will review our findings with you and discuss our recommended treatment plan. Finally, with your consent, we will begin the first treatment and start the process of restoring your body back to health.

Normally the doctor will recommend a course of  treatments and will recommend to see you again for your second visit within a few days. This is to monitor your progress and see how well you are responding to care. As we work with you in the healing process it is important to follow the proposed plan so that you can get the appropriate care you need. The doctor may adjust or change your treatment plan as you progress through the healing process depending on your individual needs. We may also make exercise, nutrition, or lifestyle recommendations that will help you achieve your health goals.

Although many of our patients initially visit us for a specific cause or reason, they often choose to continue to receive treatment after the experience a relief of their initial symptoms. By doing this, these patients graduate from receiving acute and rehabilitative care to maintenance and wellness care. The goals of preventive and maintenance care are to maintain proper health and function and prevent further injury. Research has shown that Chiropractic care has been extremely effective when employed in this capacity. True health and wellness in not just the absence of symptoms and disease, but optimal function of physical, biochemical, and emotional well-being. In choosing to receive wellness care, these patients are seeing the daily benefits of optimal function of their bodies.
